Avalon is a six bedded specialist community home, focusing on the short and long term rehabilitation of individuals with learning disabilities and past or present mental health issues. The home is easily accessed by rail or road from all major towns and cities and is located close to the centre of Cardiff .
The Home
Avalon is a Victorian property set over three floors with paved areas to the front and the rear. The home has 6 individual bedrooms that are spacious, well-furnished and decorated to individual taste. Communal areas include the kitchen, dining room and two lounges providing a comfortable and homely living environment. Approximately a two minute walk away from local pharmacy, post office, newsagent and small supermarket.
The Care
There is a dedicated staff team on duty 24-hours a day to provide support.The highly trained team are committed to providing a safe environment and facilitating independence and dignity through the development and review of individual care plans. Avalon promotes Craegmoors’ communication strategy called “Your Voice” which ensures that each individual, based on their preferred method of communication, is able to have a voice on how their supported care is provided. Monthly service user meetings are held where individuals are able to represent the views and voices of fellow service users. This is extended to a voice at area, divisional and national level. Avalon liaises with families, community organisations, social services departments and purchasing authorities to support Person Centred Care Planning, and ensures full account is taken of social, emotional, cultural and spiritual needs of all individuals.
The Facilities
Avalon provides a wide range of client centred activities, both within the home and in the community. A well established day care programme is operational with clients involved in training for daily living skills. Arts & crafts and gardening are just some of the activities our clients are involved in. Opportunities are available for people to attend local colleges and educational/recreational centres within the local community. Social activities include cinema visits, day trips and pub outings.
