Independent Hospital
Ty Gwyn Hall Independent Hospital provides 24 hour nursing care to individuals diagnosed with a mental illness. The hospital is registered to take both male and females, who may be detained under the Mental Health Act 1983. Care is provided across three individual units and in total we can provide 34 beds. The hospital also has priority access to a community residential placement in the local town.
Overview of service
The hospital provides high levels of staffing throughout its services so although care is provided within an open setting we can offer high levels of relational security should the needs of the individual demand this.
Skirrid View provides 12 individual en-suite bedrooms. Individuals undergo an assessment period in this newly built facility, in order to agree an individualised person centred rehabilitation programme. After an initial six week assessment period we will convene a review meeting to discuss and agree a care pathway specific to people’s needs.
Following a sustained period of engagement and participation within the assessment unit, the move to Ty Gwyn represents the next step forward towards recovery. Ty Gwyn Hall accommodates 18 people in a fully refurbished mid Victorian country house. Within this unit, people will continue to work at all aspects of their rehabilitation with emphasis on maximising independence.
When individuals are evidencing sustained progress and self direction in their treatment and care pathway, the opportunity will be discussed for their move to our newly developed four bedroom ‘Step-down’ unit, Pentwyn House. Here, individuals are expected to further enhance their activities of daily living, and engage in vocational and educational opportunities in preparation for their successful transition to the community.
Brynawel is our latest innovation, centrally located in Abergavenny. This 24 hour staffed house offers four people the opportunity to live independently while retaining the full support of the Multi-Disciplinary Team from the main hospital.
Ty Gwyn Hall independent Hospital employs a robust and experienced team of multi-disciplinary professionals. Our team includes Responsible Clinicians (RC’s), Clinical Psychology, Health Psychology, Social Work, Occupational Therapy and Registered Nurses. This team is also complemented with regular input from registered Art and Music Therapists who can offer our individuals Music Psychotherapy, Art Psychotherapy and Guided Imagery with Music.
Through the use of a holistic and person centred approach our experienced multi disciplinary team encourage everyone to exercise choice and work toward independence. Utilising the Care Plan Approach we will work closely with the individual, their care co-ordinators and commissioning teams in delivering a package of care that is realistic, specific to need and will achieve the best possible outcome.
Ty Gwyn Hall Independent Hospital has been subject to an extensive refurbishment programme which has not only allowed for the development of new accommodation but has updated all of the existing facilities. All bedrooms offer single accommodation in spacious personalised rooms.
The hospital benefits from three large occupational therapy rooms. Each area is equipped to meet the needs of those who use our service, and provides an environment suitable for both open and closed therapy sessions.
What it's like to live here
A significant part of the work that we do at Ty Gwyn Hall is to encourage those who use our service to engage with educational, vocational and quality of life opportunities. Leisure activities such as fishing, cinema, hill walking and horse riding are regularly planned and facilitated. Our team within the Occupational Therapy department will be pleased to work with any of the individuals on a one to one or group basis, engaging them with planned activities they have chosen as part of their rehabilitation.
Our Occupational Therapy Department is guided by the Model of Human Occupation. This practice model and it’s tools focus on a person’s motivation for occupation, their occupational behaviour (habits/routines) performance skills and on how environments influence all of the above to assist individuals to engage in meaningful occupations of their choice.
We endeavour to provide a therapeutic environment that is conducive towards the rehabilitation of those who use our service and maximise their independence.
Surroundings
Ty Gwyn Hall is located in substantial grounds within the Brecon Beacons National Park. It has superb views of the surrounding countryside and is close to numerous areas of natural beauty.The market town of Abergavenny is nearby and has a wide array of shops along with leisure centres, public swimming pool and a regular weekly market. Cardiff, Newport and Hereford are also a short journey away and we can provide transport and support to enable individuals to attend music, sports and other leisure events that are held at venues in these towns and cities, if appropriate. There will however, be an emphasis on assisting people to use public transport and develop their independence.
Abergavenny has excellent bus and rail links along with being close to both the M4 and M5 motorway networks that allow easy access to all parts of the country.Ty Gwyn Hall has excellent contacts with several educational and work placements in the locality. A number of our individuals will attend these placements as part of their rehabilitation programme.
Service facilities
Ty Gwyn Hall prides itself on promoting a relaxed yet therapeutic environment. The main manor house was built in the late 1800’s and has recently been fully refurbished whilst the other units on the site were built in 2008 as part of a major development by Craegmoor. Each of our units are decorated and maintained to a high standard whilst all bedrooms provide single person accommodation with all having en-suite facilities. Everyone is encouraged to personalise their rooms and can utilise their own furniture if they so wish.
The hospital has three vehicles with which to provide transport and ensure that leisure activities, appointments and home visits are facilitated as planned.
Our busy Occupational Therapy department will work with individuals to create a plan of activities which will include a balance of their wishes and their rehab needs. Activities such as fishing, cinema trips and bowling are regularly organised. If somebody has a particular hobby or past-time that they enjoy doing we will endeavour to make sure that this is provided.
The hospital is set within its own substantial grounds and includes a landscaped sensory garden and areas to grow their own flower beds and vegetable patches. We hold regular barbeques in the summer and have several areas where games such as cricket and football can be played.
We have also fully updated our kitchen facilities and our team of dedicated Chefs and kitchen staff will provide home cooked, nutritious meals which reflect their dietary and cultural needs. Individual’s can also utilise our Occupational Therapy kitchens to plan, budget and prepare their own meals as part of their rehabilitation programmes.
